Our Approach

Our Approach

How We Work With Existing Systems

How We Work With Existing Systems

Island Echoes Health is informed by lived experience, community input, and research focused on health equity and system navigation.

Our approach prioritizes:

  • Local context and on the ground realities
  • Transparency and accountability
  • Collaboration with clinics and community providers
  • Continuous learning and adaptation

We believe meaningful change happens when solutions are shaped with communities, not imposed on them.

How We Work With Existing Systems

How We Work With Existing Systems

How We Work With Existing Systems

Island Echoes Health does not replace public healthcare, insurance programs, or government services.

We support existing systems by:

  • Helping patients and families understand what services are available
  • Coordinating care across public and private providers
  • Improving communication between clinics, families, and caregivers
  • Reducing delays that lead to emergency care

Our goal is to help people use the healthcare that already exists more effectively.

Companion Care Support

Medical Services and Care Coordination

Medical Services and Care Coordination

Island Echoes Health includes a companion care component for elderly adults and individuals with mental or cognitive disabilities.

Companion care focuses on:

  • Regular check ins and wellness monitoring
  • Appointment reminders and visit support
  • Help navigating referrals and next steps
  • Communication with families when care is delayed or missed

This support is designed to reduce isolation, improve follow through, and help vulnerable individuals navigate care more safely.

Medical Services and Care Coordination

Medical Services and Care Coordination

Medical Services and Care Coordination

Island Echoes Health supports access to medical services by helping patients and families coordinate care across clinics, hospitals, laboratories, and pharmacies. We do not provide medical care directly. Instead, we help ensure that care is scheduled, completed, and followed through.

Our medical services focus on improving continuity and reducing missed or delayed care.

Medical Services Support Includes:

  • Coordination of primary care visits and routine check ups
  • Support for chronic disease management such as diabetes, hypertension, and asthma
  • Assistance navigating referrals between public hospitals, private clinics, laboratories, and pharmacies
  • Tracking of completed visits, tests, and follow up appointments
  • Communication with families when appointments are completed or missed
  • Support understanding next steps after a visit, test, or referral

Why This Matters

Delayed and fragmented care places unnecessary strain on patients, families, and healthcare systems. By improving coordination and follow up, Island Echoes Health aims to support earlier intervention, better continuity of care, and reduced pressure on already stretched facilities.
